The International Classification of Diseases, 10th Revision, Procedure Coding System (ICD-10-PCS) is a U.S. cataloging system for procedural codes that track various health interventions taken by medical professionals.

ICD-10 replaced the ninth revision of the system in 1993, and ICD-10-PCS is a U.S.-specific clinical modification of the original ICD-10. The ICD-10-PCS code set is used in inpatient and hospital settings.

The updated ICD-10-PCS code set has more codes than the ICD-9 version, and this helps support current health information needs. ICD-10-PCS codes must be used on all Health Insurance Portability and Accountability Act (HIPAA) transactions.

Code structure

ICD-10-PCS is developed by the Centers for Medicare and Medicaid Services (CMS) in conjunction with 3M Health Information Management to track international morbidity and mortality statistics in a comparable way.

The system uses three- to seven-digit alphanumeric codes to specify medical procedures. The first digit indicates the section of medical practice (surgery, administration, measuring and monitoring, etc.) and the following digits specify the body system, root operation, body part, approach and the device used. The seventh character is a qualifying digit.

The crucial first three digits of a code are stored in the ICD manual for reference. For example, a code beginning in 0C0 would be a medical/surgical procedure on the mouth or throat -- specifically, an alteration.

Here is an example of what an ICD-10-PCS code looks like: 047K0ZZ.

ICD-10-PCS vs. ICD-10-CM

The U.S. also uses ICD-10-CM (Clinical Modification) for diagnostic coding. The main differences between ICD-10 PCS and ICD-10-CM include the following:

  • ICD-10-PCS is used only for inpatient, hospital settings in the U.S., while ICD-10-CM is used in clinical and outpatient settings in the U.S.
  • ICD-10-PCS has about 87,000 available codes while ICD-10-CM has about 68,000.
  • An ICD-10-PCS code can be made up of any combination of numbers and letters while with ICD-10-CM, the first digit has to be either a number or letter and all other digits are numbers.

ICD-10-PCS guidelines

Approved by the four organizations -- the American Hospital Association, the American Health Information Management Association, CMS and the National Center for Health Statistics -- the guidelines for ICD-10-PCS are a set of rules that have been developed to accompany and complement the instructions provided within the coding system. However, the instructions provided within ICD-10-PCS take precedence over these guidelines.

The guidelines provide additional instruction to the coding and sequencing instructions in the tables, index and definitions of ICD-10-PCS.

The purpose of these guidelines is to assist healthcare providers and coders to identify the procedures that need to be reported.

Changes to the ICD-10-PCS code set for 2017

All the changes made to the ICD-10-PCS guidelines for 2017 occurred in the Medical and Surgical Section Guidelines.

Changes were made to the Body System General Guidelines section, Root Operation (removing a body part) Guidelines section and more.

Since the time we have come to know that ICD-9-CM will be replaced by ICD10 codes in future, we are more eager to learn these new codes. For the diagnosis part, that is Volume 1 and 2 part of ICD9 is same compared to ICD10 because the guidelines remains the same. The only difference will be the use of new alphanumeric codes in ICD10. Until now, everyone has only thought about diagnosis what about ICD Volume 3 procedure codes?  So, here are the main challenges who don’t know What is ICD-10-PCS coding. ICD1-10-PCS is a combination of alpha and numeric to form procedure codes to provide a coding system of medical classification to determine payment and support EHR (electronic health record) for all the inpatient procedure performed in United States. The codes are designed in such a way that the coders can filter the codes on the basic of section, root operation, body part, approach, device etc. to reach out to the correct procedure codes. First let use checkout how these codes are different from ICD 9 volume 3 codes.

1. ICD-CM has only Numeric for Procedure codes but ICD-10-PCS codes are alphanumeric
2. ICD-9-CM are normally 3 to 4 digits long but ICD-10-PCS has seven compulsory characters to code any procedure.
3. ICD-9-CM follow diagnosis coding structure but iCD-10-PCS designed to meet healthcare needs for a procedure code system

ICD-10-PCS codes, (International Classification of Diseases, 10th Revision, Procedure Coding System) is the new procedure coding system developed by 3M as a replacement for ICD-9-CM, volume 3.  These are used as diagnostic codes, serving as a tool for reporting basic health statics, as well as for indexing/ reporting the following:

The ICD-10-PCS manual is divided into an

index, followed by 17 sections of code tables. 

Index

The Index is located at the beginning of the manual and is an alphabetical list of terms,

which can be used to locate the appropriate

code table. The main terms include root operations, root procedure types, or common names, and thus the index reflects the

structure of the table. 

The index is based upon the third character, lists anatomic sites, uses device terms, and contains common procedures. It will always provide at

least the first three or four values of the codes, while it will sometimes provide the entire code. 

It should be noted that the index is used ONLY to assist in locating the tables where the codes are constructed and it does not have to be consulted before proceeding to the tables.  

Code Tables 

The ICD-10-PCS is divided into 17 sections of Code tables, which are organized by general type procedure. The largest of these sections-- The Medical and Surgical Section-- contains 31 body system values and 31 root operations.

The first three characters of procedures define each table, while the columns provide the last four characters and rows providing values for each character. 

Code Format 

ICD-10-PCS codes are consist of 7 alphanumeric characters. ALL seven characters MUST be specified in order to be a valid code. A character specifies the same type of information in that particular axis of classification when within a defined code range. For each axis, one of 34 possible values can be assigned, including numbers 0-9 and letters of the alphabet, sans the letters "I" and "O".  (These letters are not used, as they are easily confused with numbers "1" and "0.")
